Name Consistency Conventions

To avoid problems or delays in obtaining your visa and entering the United States, make sure your name is entered correctly and consistently on all documents:

  • Ensure that all application and immigration documents reflect the same name, exactly as it appears in your passport
  • Do not use nicknames or shortened names on any of your documents, including your application, passport, DS-2019, entry visa, I-94 card (arrival/departure cared that is given out and completed in the airplane) or any additional documents that you acquire after arrival in the United States
  • If you have more than one last name (for example, your mother's and father's last names), be sure to put both names in the Last Name field. Do not put either last name in the Middle Name field, or your visa documents will be incorrect
